Unless your presenting complaint was about your weight, your doctor shouldn't be assuming you want to lose weight and giving you medications to accomplish it. If you're 5'8″, 158 isn't that high. If you do want to lose weight, more exercise and smaller portions is the way to start off. I dislike prescription medications for weight loss in general. My main question for you though would be “do you feel like you would like to lose weight?”

Yes you will because with low insulin your fat-cells become for all intends and purposes one-way streets. Your body will use fat but not store it. Any dietary surplus will simply be “evacuated” while shortfall (such as in the middle of the night) will be taken from stores. Unless you keep eating just as you spend energy you’ll not avoid weight-loss. Leaving aside this tedious energy balance idiocy, high fat diets also are more filling and have a metabolic advantage from the get go while decreased insulin will get rid of the food gravings that are typical of high-carb diets. So actually every single thing we know about this says that people on a keto diet are very likely not overeating. So what if they follow that advice to cut calories? They’ll cut fat and they’ll feel weak and they’ll be hungry again. All for nothing.

Still safe, IMO. Once the alcohol is used up in a matter of hours, it’s straight back to ketosis with no other metabolic side effects. A couple hours of your liver being busy with alcohol won’t stall any weight-loss efforts.
